✨ feat(mcp): 新增运行期失败诊断探针

- 解析 gonavi.log 中 MCP 启动、发现和调用失败信号

- 结合已保存 MCP 服务与工具发现状态输出原因和 nextActions

- 补充系统引导、工具目录、状态标签和回归测试
